In order to make your visit to Lafayette General Endoscopy Center pleasant and comfortable for you and your designated driver, we are providing the following information. Please call us at 337-289-8249 if you have any additional questions.
· There are some registration forms that will need to be filled out prior to your procedure. The process will take much less time if you have done the paperwork prior to your arrival at LGEC. If you received a packet at the physician's office when your procedure was scheduled, there is a registration packet included. Please fill the forms out and give the completed packet to the receptionist when you arrive for your procedure at the center. If you didn't receive a packet from your physician, the forms can be downloaded by clicking the "New Patient Forms" option below. You can also call us and we can fax them to you or you can pick them up here at the center. Our hours of operation are Monday thru Friday - 7:30 AM to 4:00 PM – Closed on Holidays. If you have been here previously within the last year, you will not need new forms unless there has been a change in the information (change of address, different insurance, etc.) that was listed. Bring your driver's license or pictured ID and your insurance cards with you – we need to make copies for your chart.
· Make sure that we have a phone number where you can be reached on the day of your procedure. We may call you to change the time of your arrival if the schedule changes due to a cancellation or delay. Unless you are contacted, arrive at your regularly scheduled time.
· You must bring someone to drive you home when you are discharged. You will not be able to drive until the following day due to the sedation you will receive for your procedure. Your driver does not have to stay here and wait for you – they can leave a cell phone number and we will notify them when you are in the recovery room. Your driver should be within15 minutes of the center when notified.
· Only bring one person with you for your appointment. We often have more than one physician working at the center at the same time. This means there are twice the number of patients and drivers waiting and it can become crowded in the waiting room.
· We do not allow food or beverages in the waiting room. First and foremost, our patients have been fasting for several hours, and we do not want people eating or drinking in front of them. We also do not want spills and leftover food and wrappers on the carpet and furniture. There is a vending and sitting area on the second floor of our building and also a coffee shop and cafeteria on the first floor of Lafayette General Medical Center, which can be accessed by the skywalk from this building to the hospital.
· In consideration of others, we ask that patients and visitors put cell phones on vibrate and step into the hall or lobby for phone conversations. There are no cell phones allowed in the patient care areas.
